Our Referral Process for Case Management Services
We welcome enquiries from funders, families, solicitors and other professionals.
Initial Enquiry
Whether you need a one-off report or ongoing case management, our referrals team reviews your request and recommends.
One-off Reports
We provide:
- Immediate Needs Assessments (INAs) under the Rehab Code
- Specialist reports, including Safeguarding Reports, Funding Reports and Mental Capacity Assessments
Standalone reports may attract VAT; reports completed within case management do not.
Case Management
If you require case management, we send case manager profiles for approval by all parties. Once approved, we schedule a free online ‘meet and greet’ so the client can ensure the case manager is a good fit.
Assessment Visits and Reports
We provide a clear cost and timescale before starting.
For case management, an inperson initial assessment at the client’s home explores their goals, as well as their physical, cognitive, social, emotional and behavioural needs. The resulting report includes:
- A tailored case management plan
- Predicted implementation costs
- Recommendations for services, equipment and support
Starting Case Management
Once you confirm agreement to the plan and costs, we begin implementing case management services.

Case Study
Enabling a life of comfort, connection, and meaning after severe injury
AB, a highly creative and skilled craftsperson, sustained a severe traumatic brain injury in a road traffic collision in 2016 when she was in her 30’s and had a young child. After lifesaving treatment and 18 months in a prolonged disorder of consciousness unit, she was assessed to be in a minimally conscious state with complex physical, cognitive, and medical needs.

Summary of injury and rehab pathway
RTA in 1988 followed by specialist rehabilitation for 2-3 years, moving on to a longer stay accommodation at the rehab unit. At his request he then moved to his own house with minimal support until a gradual deterioration led to an emergency hospital admission in 2015, as which point case management was sought by his Deputy.
