Enabling a life of comfort, connection, and meaning after severe injury

AB, a highly creative and skilled craftsperson, sustained a severe traumatic brain injury in a road traffic collision in 2016 when she was in her 30’s and had a young child. After lifesaving treatment and 18 months in a prolonged disorder of consciousness unit, she was assessed to be in a minimally conscious state with complex physical, cognitive, and medical needs.

Presenting Needs

When case management began, AB was still hospitalised and required complete support with all aspects of daily living, intensive medical oversight, coordinated rehabilitation, specialist equipment, and a longterm plan that could eventually enable safe, dignified care at home. Her family also needed consistent guidance and support.

Our Approach

Case management coordinated every aspect of AB’s rehabilitation and care from 2016 to 2026. This included her transition through specialist rehabilitation services, the full adaptation of her flat, development of a bespoke 24/7 twotoone support package, and ongoing multidisciplinary input at home, spanning physiotherapy, speech and language therapy, occupational therapy, neuropsychology, and sensorybased work such as music therapy.

We developed structured routines, communicationfocused interactions, and tailored sensory interventions that helped AB express preferences, maintain comfort, and experience meaningful engagement. Medical management, emergency planning, documentation, and continuity of care were consistently overseen to ensure her safety and wellbeing.

Crucially, the care model actively supported AB’s family to remain closely involved in her daily life, enabling strong emotional connections and familiar, comforting experiences.

The Outcome

With a stable, carefully coordinated support system, AB enjoyed:

  • meaningful responses to music, voices, and sensory stimulation
  • emotional connection with her son and wider family
  • calmness and wellbeing through consistent routines
  • ongoing physical stability through proactive management

AB lived at home, surrounded by the people and environment she loved, until her peaceful passing in February 2026. Her care journey reflects how holistic case management can create comfort, dignity, and connection even in the most complex circumstances.
